Postępowanie w zespole zaburzeń oddychania (ZZO) u noworodków zmieniło się znacznie na przestrzeni ostatnich lat. Optymalna opieka nad wcześniakami rozpoczyna się już w okresie prenatalnym, w którym niezwykle ważnymi elementami są transport do ośrodka III stopnia referencyjności oraz odpowiednia steroidoterapia prenatalna. Po porodzie istotne jest postępowanie na sali porodowej zgodne z aktualnymi rekomendacjami, a zatem uwzględniające opóźnione odpępnienie i wczesne wspomaganie oddychania metodą nieinwazyjną. Kluczową rolę odgrywają podaż surfaktantu, ścisła kontrola podaży tlenu w mieszaninie oddechowej oraz unikanie intubacji i wentylacji mechanicznej. W artykule przedstawiono aktualne standardy leczenia ZZO na podstawie wytycznych europejskich ekspertów w dziedzinie perinatologii i neonatologii, opublikowanych jako European Guidelines on the Management of Respiratory Distress Syndrome: 2022 Update.

Abstract:
Management of the respiratory distress syndrome has changed over the years. The optimal care includes appropriate maternal transfer to a perinatal center and appropriate use of antenatal steroids. Protective management in the delivery room is crucial, with delayed umbilical cord clamping and non-invasive respiratory support from birth. In the intensive care unit, the treatment incorporates surfactant administration, judicious use of oxygen, and avoidance of intubation and mechanical ventilation where possible. The article presents the actual standard of care for RDS, based on the European Guidelines for the Management of RDS: 2022 Update, prepared by a panel of European neonatologists and perinatal obstetricians.
